Welcome Wagon: After joining West Regional Physicians Network through a Medicare Advantage Plan, each West Regional Physicians Network member will be visited by a clinician who will conduct a wellness exam, provide guidance and answer questions.

Comprehensive Care Clinic and Immediate Care Center: Our Downey Comprehensive Care Clinic features all-day walk-ins and is available for your primary care needs when a doctor is not available. In addition, we are supporting key specialties like Dermatology, Ear Nose and Throat, and Gastroenterology. In addition, we feature useful programs such as:

Hospital Discharge Clinic: West Regional Physicians Network Medical Group providers and pharmacists are on site to meet with patients and review their hospital stay, medications, and work together to ensure that they have everything they need to be able to support their transition home from the hospital.

Coumadin Clinic: At our Coumadin Clinic, our clinicians work together with patients to monitor their blood thinning medications and answer any questions they may have regarding this treatment.

Activity Center: Our Activity Center is right across the hall from our Clinic and offers our members educational and recreational events on a weekly basis. Visit (events page) for more information. An West Regional Physicians Network pharmacist and dietician each host monthly talks at the Activity Center to educate patients on how best to care for their own health. Social Service Programs: Our Social Services department is available to help members find resources that might be available in the community to help manage their overall health. Telehealth: We offer discounted rates on Amwell Telehealth. This allows members to speak to a doctor from the comfort of their own homes using the Amwell app.
We understand that you may have questions about what hospice care means for you and your loved ones. We are here to help. West Regional Physicians Network provides emotional, spiritual and clinical support toward the end of life. We are there to support the whole family. We offer this care whether you are at home, in an assisted living community, in a skilled nursing facility, or in a hospital. Our hospice care team will work with you and your doctor to develop a care plan that will help you find comfort and preserve dignity — all in a peaceful setting. Supportive Care for You Wellness and Supportive Care is extra care for you at home. Living with a chronic health condition can be tough. Our dedicated Supportive Care team will work with you and your doctor to make sure you can enjoy your daily activities at home and achieve your wellness goals. If you experience any change in your symptoms, you now have 24/7 access to a team who can help you right away. We can also assist you with: Establishing health and wellness goals Scheduling doctor appointments and transportation Coordinating home services, including nurse visits, physical therapy, etc. Getting medications and medical supplies delivered to your home Who Can Benefit Supportive Care services are for patients who need specialized medical care for chronic health conditions. Care Management is an important service for West Regional Physicians Network members who need a little extra help managing their healthcare.  Our Care Management team makes sure that you have all of the support you need to stay healthy and get the care you need! 

 

Who We Are

Our Case Management team is made up of experienced Nurses and Case Management Assistants: 

  • Nurses – Registered Nurses (RN) or Licensed Vocational Nurses (LVN) with special training and experience in caring for seniors and members with multiple medical conditions. 
  • Care Management Assistants – Skilled members of the West Regional Physicians Network team who can assist you with finding a specialist, making doctor’s appointments, and other needs you may have. 

What We Do 

Your West Regional Physicians Network team will work with you, your family and your doctors to develop the Care Plan that supports your medical and social needs. Some of the services we provide include 

  • Education on disease management and preventive care 
  • Education on how to take your medications 
  • Help making doctor’s appointments 
  • Help getting community resources 
  • Advanced care planning 

Who Should Enroll for Care Management Services? 

  • Members with complex healthcare needs or multiple healthcare issues 
  • Members that have gone to the hospital or emergency room multiple times recently 
  • Children with complicated healthcare issues 
  • Potential transplant patients (bone marrow, heart, kidney, liver, lung) 
  • Members who would like extra support managing their healthcare

AppleCare is dedicated to offering the latest in preventive care. Your primary care provider will recommend services to you considering the recommendations provided by the U.S. Preventive Services Task Force.

  • May I Continue Seeing My Doctors and Specialists? Yes. We encourage you to continue seeing your doctors. The Diabetes Care Team will work with your doctors to support your Care Plan.
  • What Should I Bring to my First Appointment? Please bring your glucometer, medications, glucose logbook and anything else you think would be helpful.
  • What Should I Expect at my First Appointment? At your first appointment, we will be doing a thorough assessment, including an HbA1c measurement onsite. Together we will create a plan individualized to your specific needs.
